The wait is almost over for fans of Ice-Pick Lodge's haunting survival RPG series. The studio has officially announced that Pathologic 3 will launch on January 9, 2026, bringing players back to the eerie, plague-ridden world that defined its predecessors. To make things even better, there's a new free demo available right now on Steam as part of the Steam Next Fest.

Surreal psychological horror at its best

The announcement came alongside a new trailer that perfectly captures the unsettling tone the series is known for. Bleak environments, cryptic dialogue, and an ever-present sense of dread are the highlight of a game that follows the steps of its predecessors, but adds some novelties. Pathologic 3 once again follows Daniil Dankovsky, the Bachelor, a doctor desperate to save a doomed town gripped by disease and decay. As before, players will have just twelve in-game days to uncover the town’s secrets, manage limited resources, and decide who lives and who dies.

One of the biggest new twists this time around is a time-jump mechanic. Players will reportedly be able to revisit past moments to change key decisions. However, knowing Ice-Pick Lodge, those changes are bound to come at a price. The game promises to expand on the complex moral systems and survival elements that made Pathologic 2 both unforgettable and punishing.

To give fans a taste of what’s coming, Ice-Pick Lodge released Pathologic 3: Quarantine, a free prologue demo. Now, another version of the demo with additional content is available on Steam now live on Steam. It serves as a self-contained introduction to the game's world and systems, offering a few hours of gameplay filled with tense encounters, medical diagnoses, and moral dilemmas.
